Evo 8 LSD question

I've been changin the LSD of my brothers evo cause the bearings of the bevel gear failed.
After 8 miles the new one failed again, any ideas why?

You guys running staggered offset or different size tires?

center or rear diff? and what Panem said... offset doesn't matter much, but staggered tire sizes on an EVO or STI can be bad. I don't see anything fitment related causing a failure after 8 miles though. There's definately something else going on.
